Here is the 2008 Team Core Hawaii application and waiver form. If you are a returning member and your personal information is the same as last year, you do not need to fill out a new application, just fill out the waiver form.

Make checks payable to TEAM CORE HAWAII and mail to P.O. Box 26281, Honolulu, HI  96825. Once your payment is received, your 2008 membership card will be mailed to you.

Reminders: Team Core members are entitled to 20% discounts on all regular priced running, cycling, swimming, triathlon merchandise at McCully Bicycle.
